We are excited to announce the Population Health Management Collaborative’s (PHMC) summer Member Meeting, taking place in Nashville, TN on June 18-19, immediately before Premier’s 2018 Breakthroughs Conference and Exhibition! The meeting will focus on building a successful I.T. model for population health.

Effective informatics and technology are vital capabilities required for success in value-based payment and care delivery models.  Premier has verified this to be true through its exposure to numerous provider organizations across the country, which is why informatics and technology is one of the six core components included Premier’s PHMC Capabilities Framework. Through the PHMC we have learned that optimizing this component is a complex and arduous process.  Some common challenges include, determining what is most important and where to invest, learning and optimizing immature and expensive I.T. solutions, incomplete and low-quality data, and identifying solutions that continue to meet the needs of an evolving organization.

Recognizing the importance of this topic and the associated challenges, this meeting will provide a unique opportunity to collaborate, share best practices, and learn from several other healthcare delivery systems.  It will focus on identifying tactics that maximize EHR platforms and exploring emerging technologies that are enabling health systems to improve care and performance under value-based models.

The meeting will feature presentations from high performing ACOs, subject matter experts, and I.T. vendors.  The meeting will also include workgroup sessions that foster sharing and collaboration.  The speakers and breakout sessions will focus on identifying and sharing tactical solutions and tips for success.

Don’t miss this unique opportunity to collaborate, learn, and explore this important area of Population Health Management.

We are excited to announce the launch of the Physician Enterprise Collaborative (PEC) and its inaugural meeting in Nashville, TN on June 18-19, immediately before Premier’s 2018 Breakthroughs Conference and Exhibition! This collaborative is a facilitated forum that will drive enhanced medical group performance through analytics, collaboration, and measurement of outcomes that can set trends and influence healthcare policy. It provides a forum and means for leaders of medical groups to tackle common challenges like reducing medical group investment, increasing physician alignment, changing operating models for tomorrow, redesigning physician compensation plans, and integrating academic and community medical groups. The meeting will focus on how members can act as internal disruptors, the care teams of tomorrow, knowledge sharing of lessons learned from leading organizations and provider health.

The session will be held at the same facility as Premier’s 2018 Breakthroughs Conference and will adjourn just a few hours prior to the official kickoff of Breakthroughs, allowing the PEC members to stay and take advantage of all that the conference has to offer.
