We are excited to announce the PHMC’s summer Member Meeting, taking place in Washington D.C. on June 26-27, immediately before Premier’s 2017 Breakthroughs Conference and Exhibition! The meeting will focus on best practices for ACO success and feature lessons learned from top performing ACOs. Based on the most recent Medicare ACO performance data released, PHMC members had higher average quality scores, and 50% of PHMC members achieved shared savings compared to 31% nationally.

Recognizing this success and the need for ACOs to build upon their progress, this meeting will focus on key capabilities, common challenges, and tactical, leading practice solutions that have been proven to drive success for ACOs across the country. The meeting will be designed to explore challenges and solutions for established ACOs, ACOs that are in their beginning stages, and health systems that are currently evaluating participation in a Medicare ACO.

The session will be held at the same facility as Premier’s 2017 Breakthroughs Conference and will adjourn just a few hours prior to the official kickoff of Breakthroughs, allowing all PHM Collaborative members to stay and take advantage of all that the conference has to offer.
